Baptism Certificate
To request a copy of a baptism certificate, please email Maria King. Please include the name of the person who was baptized, the date or at least the year the sacrament was received, if possible, and the name of the parent(s). We also ask that you include the preferred contact info for the person requesting the certificate. If the certificate needs to be sent to a church, please include the name of the church, the address with city, state, and zip code for the church, the phone number of the church, and the way the church would like to receive the information (mail, email, fax, etc.).
Confirmation Certificate
To request a copy of a confirmation certificate, please email Maria King. Please include the name of the person who was confirmed, the date or at least the year the sacrament was received, if possible, and best contact information for the person requesting the certificate. If the certificate needs to be sent to a church, please include the name of the church, the address with city, state and zip code for the church, the phone number of the church, and the way the church would like to receive the information (mail, email, fax, etc.).
Godparent or Sponsor Letter Request
To request certification to become a godparent at a parish other than St. Matthew, please email Laura Hollowell. Please include in your email your name, your preferred contact information, the name of the person for whom you are requesting to act as godparent, the name, address, and phone number of the church where the child will be baptized, the method which the church would like to receive your certification (mail, email, fax, etc.), as well as the proposed date of the baptism.
